## What triggers it

Dense relationships competing for a label corridor, containment relationships with insufficient clearance, or authored label offsets that move text over protected content. The warning names the affected interaction.

## How to fix it

Remove the conflicting label offset, increase available space or split the view. Persistent occlusion without authored offsets is a renderer defect. Verification fails because retained metadata alone does not make hidden content readable.

Run `am verify diagram.mmd --json`, inspect this code, and apply the smallest source or typed mutation that clears it. If it persists after two mechanical attempts, return the warning and ask for human review.
